MDS Coordinator Job Description
Position Overview:
We are seeking a highly motivated and dependable MDS Coordinator to join our team. The ideal candidate is a registered nurse with MDS experience, but we are willing to provide training to the right candidate. This role is critical in ensuring accurate and timely completion of the Minimum Data Set (MDS) assessments and coordination of care plans to meet regulatory requirements and optimize patient care outcomes.
Key Responsibilities:
* Complete and submit MDS assessments according to state and federal regulations.
* Coordinate interdisciplinary team meetings and ensure comprehensive care planning.
* Monitor and document patient progress, updating care plans as necessary.
* Ensure accuracy and compliance with all documentation.
* Train and support staff in understanding and adhering to MDS requirements.
* Collaborate with nursing staff, physicians, and other healthcare professionals to deliver high-quality care.
Candidate Qualifications:
* Registered Nurse (RN) license.
* MDS experience preferred, but training is available for the right candidate.
* Strong organizational and scheduling skills.
* Excellent communication and teamwork abilities.
* Reliable and dependable with a strong work ethic.
