Job Description

Job Title: MDS Coordinator

Overview: We seeks an experienced MDS Coordinator to ensure accurate MDS assessments and compliance with regulatory standards.

Responsibilities:

* MDS Assessment and Documentation:

* Coordinate timely and accurate completion of MDS assessments.
* Develop individualized care plans based on assessment data.

* Quality Assurance and Compliance:

* Ensure compliance with federal and state regulations.
* Conduct audits to maintain accuracy and address discrepancies.

* Staff Education and Collaboration:

* Provide staff training on MDS guidelines and updates.
* Collaborate with interdisciplinary teams to coordinate resident care.

Qualifications:

* RN license preferred.
* Minimum 2 years' experience in MDS coordination.
* Strong understanding of MDS process and regulations.
* Proficiency in EHR systems and MDS software.
