Job Description

We are looking for a compassionate, empathetic person to lead our team of Community Health Workers. Not only will this person lead the team but they will work alongside community partners and agencies to identify and meet the needs of vulnerable populations in our community. Community Health Workers use the four principles of educate, navigate, coordinate and advocate to guide their work. Lived experience is a plus. This position will utilize the evidence-based Pathways Community HUB model in partnership with the Great Rivers United Way HUB to accomplish this goal.

Responsibilities :

• Oversee, guide and support a team of 3 Community Health Workers
  
• Manage a caseload of clients in accessing community resources, including
  
• healthcare, food, housing, employment and health information
  
• Network and collaborate with community agencies and organizations
  
• Be a part of the broader YMCA team of leaders advancing our mission as a non-profit organization dedicated to enriching our community by promoting physical, mental and spiritual well-being for all
  

Requirements

• High School Diploma
• Experience working with families with young children
• Supervisory or leadership experience a plus
